The Supporting Emerging Adults with Diabetes Team

This past July at the Bronx Zoo, the Supporting Emerging Adults with Diabetes (SEAD) program hosted an event for young adults with type 1 diabetes and their loved ones. It centered on community building, explored daily stressors of living with T1D, and also introduced patients and families to advanced diabetes technology. Designed to engage attendees with conversation in several small break out sessions, patients and family members were able to share their experiences living with T1D, and left the event more connected and empowered.

Created and led by Shivani Agarwal, MD MPH and Michael Greenberg, NP, we at the SEAD program offer comprehensive and compassionate care designed for young adults with T1D to address medical, psychosocial, developmental, and nutritional needs. As part of the program, young adults will also have access to our T1D-specialized dietician, social needs care coordinator, and psychologist. We have very close relationships with our pediatric colleagues to ensure that your transition to the adult healthcare system is smooth. We also offer ample opportunity to get involved in cutting-edge research and advocacy in the T1D landscape. Part of the Fleischer Institute for Diabetes and Metabolism at Montefiore-Einstein medical center, this program is based in the Bronx and has recently expanded to Tarrytown.

The SEAD program model

Our mission is to empower, educate, and create a supportive community of patients and families, and help them utilize available resources so they can effectively manage the daily and long term demands of type 1 diabetes. To achieve this, we plan to host multiple events throughout the year based on and driven by patient input and participation. We will continue to reach out to the community to provide events that encourage self-care and provide diabetes education, and also help connect patients and families with an in-person social network.

Ask to make a new patient appointment with Dr. Agarwal as a young adult with type 1 diabetes in the SEAD program. You must be between 16-35 years old.
